Position Summary

Direct Support Professional - PRN at Horizon Health and Wellness. This role involves flexible, as‑needed shifts during the holiday season, with additional opportunities thereafter.

Responsibilities

• Provide essential care and support services to individuals with intellectual and developmental disabilities.
• Assist with community integration and inclusion, offering guidance to individuals and their support networks.
• Create meaningful opportunities for participation in community activities.
• Ensure the health, safety, and overall well‑being of residents through proper supervision.
• Pick up shifts as needed during the holiday season and beyond.

Qualifications

• High School diploma or GED.
• Must be 21 years of age or older.
• Valid driver’s license with an acceptable driving record and reliable transportation.
• Ability to prepare required paperwork and document activities within contractually required timelines.
• Typing proficiency and working knowledge of computers and electronic clinical record keeping.
• Ability to successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, Prevention/Support, and Direct Care Worker (training provided if not possessed).
• Ability to operate independently with minimal oversight.
• Demonstrated respectful communication skills with participants and coworkers.
• Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance.
• Willingness to work nights, weekends, holidays, and a varied weekly schedule.

Preferred Qualifications

• Bilingual English/Spanish or English/Sign Language.
• Prior experience in habilitation or rehabilitation.
• Two years of experience in a person‑centered community integration program assisting persons with developmental and/or psychiatric disabilities.
• Very strong interpersonal skills.
