Job summary

New Transitions Inc is seeking Direct Support Professionals for part-time and full-time roles in Minnesota. You will support individuals with disabilities in daily living, assist with personal care, and help with medications in a nurturing residential setting.

The role requires a high school diploma, a valid driver’s license, and the ability to lift up to 50 pounds. Orientation and ongoing training are provided to meet Minn. Stat. 245D.

Qualifications

• High school diploma or equivalent is required.
• Must pass a DHS background study through NETStudy 2.0.
• Must be able to lift up to 50 pounds.
• Must complete DHS-required orientation and annual refresher training.
• Must serve as a mandated reporter under Minnesota law.

Responsibilities

• Provide direct care and support to individuals with disabilities.
• Assist with daily living activities, including personal care and medication administration.
• Maintain accurate records and documentation.
• Communicate effectively with clients, families, and healthcare professionals.
• Participate in ongoing training and professional development opportunities.
