Job summary

New Transitions Inc in Minnesota is hiring a Direct Support Professional to provide personalized care and support to individuals with disabilities. You will work closely with residents to help them achieve daily goals and maintain independence, including personal care and medication assistance, while documenting care and coordinating with the care team.

Both part-time and full-time opportunities are available.

Qualifications

• High school diploma or equivalent required.
• Valid driver's license with reliable transportation.
• No major moving violations or DUI in last 5 years.
• Must pass DHS background check through NETStudy 2.0.
• Ability to lift up to 50 pounds.
• DHS orientation and annual refresher training required.
• Must be a mandated reporter under Minnesota Vulnerable Adults Act.

Responsibilities

• Provide direct care and support to individuals with disabilities.
• Assist with daily living activities, including personal care and medication administration.
• Maintain accurate records and documentation.
