Job summary

Prairie Community Services, Inc. is seeking a Direct Support Provider in Alexandria, MN to supervise, care for and keep individuals safe in daily living activities, recreation and community orientation. Responsibilities include medication management, meal preparation, and assisting with laundry and cleaning.

Starting wage is $17.00-$19.00 per hour. Applicants must pass DHS background clearance, have a valid driver’s license, and communicate in English.

Qualifications

• Must be 17+ years old or as required by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, speak and understand English.
• Read and follow each person served’s program plans and procedures.
• Deliver support based on person-centered services.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Support daily living activities and recreation.
• Assist with community orientation and transportation as needed.
• Support medication management, meal preparation, laundry, and cleaning.
• Chart daily program information.
