Job summary

Job Description

PRIMARY RESPONSIBILITIES

Caregiver for individuals in the agency.

CHARACTERISTIC DUTIES

• Assist the individual in his/her as documented in the ISP, Service Plan etc.
• Routine hygiene care throughout the day.
• Supervise in addressing the ISP/Plan of Care goals and document progress.
• Support all opportunities for the growth, learning and development of the individual.
• Complete daily progress notes highlighting person center events.
• Complete understanding of home policies and procedures including home rules, disaster procedures, participant’s rights, etc.
• Document progress notes before leaving the job site.
• Document tracking of ISP goals prior to leaving the job site.
• Ensure the individual is integrated into the community.
• Notify the agency of any concerns regarding the individual.
• Attend all staff meeting (required), participate in scheduled training sessions including on the job training.
• Promote harmonious relationship with other staff members.
• Maintain individual’s records on site in accordance with policy and procedures.
• Other duties as assigned.
