Qualifications

• High school diploma or GED required.
• Valid State of Michigan driver’s license.
• Ability to lift 50 lbs.
• CPR/First Aid certification may be required.
• Shift flexibility: Full-Time / Part Time including weekends and holidays.

Responsibilities

• Monitor safety 24/7 with checks (eyes-on, one-on-one, 15- or 30-minute checks).
• Support crisis prevention, intervention, and resolution.
• Establish collaborative relationships built on trust and empathy through consistent communication.
• Foster partnerships with individuals served, the community, and other professionals.
• Help develop problem-solving, communication, and independence skills.
• Assist individuals with social, leisure activities and transport/escort to events.
• Assist with activities of daily living (getting to places, dressing, bathing, cooking).
• Monitor vitals and relay medical information to the care team.
• Document care plans accurately and timely.
• Support community participation and employment/career goals.
