Seven Hills Foundation is seeking a Direct Support Professional III to provide one-to-one support in a private home in Middleton, MA. The role includes helping individuals with disabilities build independence, engage in meaningful activities, and maintain a safe environment while coordinating with caregivers and the wider team.

Qualifikationen

• High school diploma/GED/HiSET required.
• Standard driver’s license – class C or class D.
• 1–3 years of experience in direct support, disability services, or related areas.

Aufgaben

• Provide person-centered support in a private home and the community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Collaborate with families, caregivers, clinicians, and team members.
• Document services and maintain confidential records in accordance with agency standards.
