Job summary

Seven Hills Foundation Inc. is seeking a Direct Support Professional III to provide one-to-one support in a private home, helping individuals with disabilities build independence and engage in meaningful activities.

You will implement Individual Support Plans, support life skills and community participation, coordinate medical appointments, and document services while maintaining confidential records; a high school diploma and a valid driver’s license are required, with 3 years of direct support

Qualifications

• High school diploma or equivalent required.
• Standard driver’s license – class C or D1.
• At least 3 years of experience in direct support, disability services, or related areas.

Responsibilities

• Provide person-centered support in a private home and the community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Document services and maintain confidential records in accordance with agency standards.
