Job summary

Seven Hills Foundation is seeking a Direct Support Professional III to provide one-to-one support in a private home in Arlington, MA. You will help individuals with disabilities build independence, engage in meaningful activities, and stay safe while partnering with caregivers.

You will implement Individual Support Plans, assist with life skills and daily living tasks, coordinate medical appointments, and collaborate with families and clinicians.

Qualifications

• High school diploma or equivalent required.
• Standard driver’s license – class C or D1.
• 3 years of experience in direct support, disability services, or related areas.
• Ability to work in private home and community settings.

Responsibilities

• Provide person-centered support in a private home and the community.
• Implement Individual Support Plans, behavior supports, and therapeutic routines.
• Assist with life skills, household responsibilities, and community activities.
• Coordinate medical appointments and support overall health and wellness.
• Collaborate with families, caregivers, clinicians, and team members.
• Document services and maintain confidential records in accordance with agency standards.
