Job summary

Momentum WORK, Inc. in the Santa Barbara area is seeking a Direct Support Professional II to provide awake overnight support to individuals living in independent and supported living settings, promoting safety, independence and dignity for persons served.

The role is part-time, Saturdays and Sundays from 3:00 PM to 11:00 PM, reporting to the Program Manager/Director of Independent and Supported Living Services.

Qualifications

• High school diploma or equivalent.
• At least 1 year of experience with persons with developmental disabilities preferred.
• Demonstrated ability to maintain confidentiality.
• Excellent verbal and written communication skills.
• Ability to complete tasks with interruptions.
• Compassion and respect for persons with disabilities.
• May need to be available during disasters per plan.
• Must have reliable phone.
• Valid CA Driver’s License with acceptable driving record.
• Valid CPR/First Aid certification.

Responsibilities

• Provide awake overnight support and coaching in daily living skills.
• Support use of community resources and maintain records.
• Assist with household tasks, budgeting, and transportation as needed.
• Prompt and assist with medication management.
• Teach adaptive living skills and monitor adherence.
• Participate in training and staff meetings.
• Assist with personal care, including medical/dental appointments.
• Refer concerns to supervisor and support social activities.
• Help participants access benefits and transportation.
• Assist with transfers as needed and crisis intervention when required.

TITLE: Direct Support Professional II,

Winchester Canyon & Goleta

Job Type: Part Time Saturday & Sunday 3:00 PM to 11:00 PM

Immediate Supervisor: Program Manager/Director of Independent and Supported Living Services

Supervision Exercised: None

SUMMARY:

Under the supervision of the Program Director and Manager, this position will provide awake overnight support services to individuals living in independent and supported living settings, promoting safety, independence and dignity for persons served.

ESSENTIAL FUNCTIONS:

• Provide individualized coaching in daily living skills
• Provide education and support in the use of community resources
• Maintain accurate and current records in ID notes and communication logs regarding information relevant to Individual Service Plans
• Support and assist persons served in following their daily activity schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks, budgeting, ledgers, and transportation as necessary
• Prompt and assist persons served in medication management
• Teach adaptive living skills and monitor the extent to which they are followed
• Participate in in-service training and staff meetings to facilitate professional development
• Assist participants in all aspects of personal care, including medical/dental appointments
• Communicate concerns related to program participant needs to supervisor in a timely manner
• Assists individuals in attending social activities to further social skills
• Train program participants to access public transportation and assist persons served in accessing benefits such as Social Security, SSI, IHSS and Section 8 housing
• Assist persons served in weight bearing and non-weight bearing transfers
• Use crisis intervention techniques in emergency situations when persons served are in danger of injuring themselves or others
• Complies with all Momentum WORK, Inc. and OSHA policies, procedures, and regulations
• Must be able to transport participants, unless waived by the Executive Director. Other related duties, responsibilities, and special projects on an as needed basis

PHYSICAL REQUIREMENTS

Ability to stand, sit for long periods of time

Ability to go up and down stairs

Ability to walk for long periods of time

Ability to push or pull wheelchairs when necessary

Ability to speak, read, hear, and understand technical written materials

The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served

Full awareness of environmental stimuli

Ability to implement Crisis Management, Safety & Pro Act techniques (training included)

QUALIFICATIONS:

• High school diploma or equivalent
• At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
• Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
• Excellent verbal and written communication skills
• Ability to complete tasks with numerous interruptions
• Compassion and respect for persons with disabilities
• Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations
• May need to be available during any disasters, per disaster plan
• Must have reliable phone
• Valid CA Driver’s License: must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and by Agency policy
• Valid certification in CPR/First Aid (Training included)

Note: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted or waived in lieu of the requirements specified above under “Education and Experience.”
