Duties and Responsibilities
- Schedule and attend medical, dental and lab appointments; ensuring coverage from other appropriate staff member when unable to attend themselves
- Notify parents/clinicians and day programs of upcoming appointments
- Maintain adequate supplies of medications and non-prescription items for residence and day programs; storing in an appropriate manner according to regulations
- Discard expired medications and supplies and complete paperwork
- Pick up medications and supplies when needed
- Check empty medication bottles and bubble packs; notifying the RN of any errors
- Assign med. certified staff (AMAP) for each shift, confirming there is at least one med-certified staff available to each shift
- Assure every AMAP passes meds in accordance with the certification requirements
- Successfully participate in the certification process to teach CPR-SFA, teaching staff as scheduled by the nurse and maintaining staff records
- Check first aid kits in the vans and residence, restocking PRNs and signing off when complete
- Check consumer specific medication information sheets and ready-to-go packets, signing off when complete
- Ensure vital signs and heights/weights are obtained as ordered
- Facilitate improved gross motor, fine motor and range of motion skills
- Promote and facilitate self-care and personal hygiene
- Assist residents with bathing, toileting, dressing and eating as needed including modeling appropriate behavior and using therapeutic equipment, if needed
- Perform housekeeping tasks (meal preparation, cleaning, laundry, shopping, etc.) when needed
- Foster receptive and expressive communication through many different avenues including sign language, pictograms and electronic devices
- Accurately and consistently complete all required documentation in a continuous manner by utilizing PrecisionCare software and in written form, when necessary
- Attend and actively participate in regular staff meetings, case reviews, in-service sessions and any other meetings scheduled or requested
- Implement fire, safety and emergency procedures, assisting with fire drills when needed
- Administer medications to residents in accordance with the Residential Medication policies and procedures
- Accurately and consistently implement behavior support plans/SCIPr
- Safely drive agency vehicles and assist in transportation to and from recreational activities and other appointments as required
- Assume shift supervisor responsibilities when requested
- May serve as an advocate for one of the residents and implement responsibilities such as writing reports, data collection, case reviews, regular classroom/day program visits, etc.
- All other duties as assigned by manager
Minimum Qualifications
- Must be at least 18 years of age
- Ability to read, write, speak and understand English including interpreting documents such as safety rules and policy and procedure manuals
- Possess a current and valid NYS driver’s license with an acceptable driving record
- High School Diploma or GED
Preferred Qualifications
- 6 months or more experience in similar role
Certifications and Trainings
- NYS Medication Administration
- SCIP-r
- CPR
- First Aid
- All other trainings mandated by the Office for people with Developmental Disabilities (OPWDD)
