Job summary

Mary Cariola Center is seeking a Direct Support Professional II - Clinical Liaison in Webster, NY. The role supports residents through orientation, training, and direct care in home and community settings, coordinating with Residential Clinical staff and ensuring adherence to program plans and reporting requirements.

Qualifications

  • Ability to participate in physical and recreational activities outside the residence.
  • Possess a current and valid NYS driver’s license with acceptable driving record.

Responsibilities

  • Assist with orientation and training of new staff in program plans and data collection systems.
  • Assist Residential Clinical staff as necessary.
  • Maintain program supplies including pictograms, schedule books, file folders, etc.
  • Monitor supply closets and ensure equipment is maintained.
  • Attend medical/health related appointments of residents as needed.
  • Organize and complete reports on documentation/data as requested.
  • Review documentation for completeness and accuracy and follow up as needed.
  • Facilitate active participation of residents by implementing their program plan at home and in the community.
  • Implement behavior support plans and SCIP-r strategies.
  • Engage in lifting, moving and positioning tasks for residents as needed.
  • Promote gross motor, fine motor and range of motion skills.
  • Support self-care and personal hygiene.
  • Assist with bathing, toileting, dressing and eating as needed.
  • Perform housekeeping tasks as required.
  • Foster communication via sign language, pictograms and devices.
  • Maintain HIPAA confidentiality and Code of Ethics.
  • Attend staff meetings and case reviews.
  • Implement fire, safety and emergency procedures.
  • Maintain certifications in Medication Administration, SCIP-r, CPR and First Aid.
  • Administer medications according to policies.
  • All other duties as assigned.

Duties and Responsibilities

  • Assist with orientation and training of new staff in all individual program plans, data collection systems, direct care responsibilities and other trainings as required
  • Assist the Residential Clinical staff as necessary
  • Maintain the necessary program supplies including pictograms, schedule books, file folders, etc.
  • Monitor supply closets and ensure equipment is maintained
  • Attend any medical/health related appointments of the residents as needed
  • Organize, collate and complete reports on any documentation/data as requested (individual program plans, active treatment plans, guidelines, etc.)
  • Review documentation for completeness and accuracy, following up with staff when necessary
  • Facilitate active participation of the residents by implementing their individual program plan in their home and community
  • Implement behavior support plans and Strategies for Crisis Intervention and Prevention (SCIP-r) accurately and consistently
  • Engage in any necessary lifting, moving and/or positioning tasks for residents
  • Facilitate improved gross motor, fine motor and range of motion skills
  • Promote and facilitate self-care and personal hygiene
  • Assist residents with bathing, toileting, dressing and eating as needed including modeling appropriate behavior, using therapeutic equipment and/or therapeutic feeding
  • Perform housekeeping tasks when needed (meal preparation, cleaning, laundry, shopping, etc.)
  • Foster receptive and expressive communication through many different avenues including sign language, pictograms and electronic devices
  • Accurately and consistently complete all required documentation in a continuous manner by utilizing PrecisionCare software and in written form, when necessary
  • Follow HIPAA confidentiality laws
  • Adhere to the Code of Ethics for DSPs
  • Attend and actively participate in regular staff meetings, case reviews, in-service sessions and any other meetings as necessary
  • Implement fire, safety and emergency procedures in accordance with agency and program standards
  • Assist with fire drills
  • Maintain certifications in Medication Administration, SCIP-r, CPR and First Aid
  • Administer medications to residents in accordance with the Residential Medication policies and procedures
  • All other duties as assigned

Minimum Qualifications

  • Must be at least 18 years of age
  • Ability to read, write, speak and understand English including interpreting documents such as safety rules and policy and procedure manuals
  • Ability to participate in various physical and recreation activities outside the residence
  • Possess a current and valid NYS driver’s license with an acceptable driving record

Preferred Qualifications

  • High School Diploma or equivalent
  • 6 months or more experience in similar role

Certifications and Trainings

  • NYS Medication Administration
  • SCIP-r
  • CPR
  • First Aid
  • All other trainings mandated by the Office for people with Developmental Disabilities (OPWDD)
