Easterseals Blake Foundation in Arizona is seeking a dedicated Direct Support Professional to assist individuals with intellectual, developmental, and physical disabilities to live as independently as possible. You will help with personal care, daily activities, and community participation.

The role involves transporting members to appointments, supporting budgeting and nutrition, administering medications as prescribed, and maintaining up-to-date ISP records.

Qualifications

• High school diploma or GED preferred.
• Caregiving experience preferred (3 months).
• Must be at least 21 years old.

Responsibilities

• Assist individuals with personal care and hygiene needs.
• Perform housekeeping tasks and home management.
• Provide transportation to appointments, outings, and shopping.
• Support community participation, budgeting, exercise, and nutrition.
• Administer medications or assist with self-administration.
• Maintain up-to-date ISP and client files.
• Report incidents and assist with incident forms.
• Provide job coaching and on-the-job training.
