Job summary

Easterseals Blake Foundation in Arizona is seeking a Direct Support Professional to help people with intellectual, physical and developmental disabilities live as autonomously as possible.

You will provide personal care, transport, and community engagement support, following each member's ISP and safety plans. This is a full-time role with comprehensive benefits and growth opportunities within a long‑standing nonprofit.

Qualifications

• High School Diploma or GED preferred.
• Three (3) months of caregiving experience preferred.

Responsibilities

• Assists individuals with personal care and hygiene needs, including restroom, eating, and mobility.
• Performs housekeeping tasks such as cleaning, cooking, laundry, and yard work.
• Provides transportation and community support for outings, medical appointments, and shopping.
• Guides community participation, budgeting, exercise, and nutrition.
• Administers medications or assists with self-administration as prescribed.
• Maintains individual files and follows ISP; reports incidents to staff.
• Assesses and applies behavior/physical management in crises; performs First Aid/CPR if needed.
• Job Coach/Direct Support: trains and supports members for work environments and job-search skills.
