Qualifications

Responsibilities

• Provide prompts to support daily activities and promote independent living, including goals, meals, cleaning, transportation, and social engagement.
• Accurately document progress toward goals and maintain logs.
• Collaborate with the DDP and local office staff to ensure quality care.
