Job summary

MMD Community Care LLC in New Jersey is seeking a Direct Support Professional to provide one-to-one support in homes or in the community, helping adults with disabilities achieve independence and inclusion.

Under the agency Director's supervision, the DSP delivers daily living assistance, medication support where applicable, meal prep, mobility help, and community activities while upholding safety, dignity, and confidentiality at all times.

Qualifications

• Female DSP preferred.
• High school diploma or equivalent.
• Valid New Jersey Driver’s License (driving record not to exceed 5 points). If transporting consumers, proof of automobile insurance is needed.
• Must be 21 years or older.
• Must clear any State/Federal criminal background checks, the Central Registry and CARI database.
• CPR/FA certification.
• Must be proficient in English (read, write & comprehend).
• Complete all training for the position within established timelines.

Responsibilities

• Assist consumer in development of community living skills.
• Assist consumer in preparation of meals, maintaining a household, toileting, dressing, grooming, and personal maintenance and opportunities for participation in the community.
• Plan and participate in recreational activities; identify outside interests and events.
• Develop, foster, and maintain relationships with family and friends; promote independence.
• Provide supportive assistance in direct residential care including medication administration if applicable; grooming; hygiene; meals; cleaning and laundry; mobility; budgeting.
• All personal health and safety needs; maintain confidentiality of consumers.
