Job summary

The New York Foundling seeks a Direct Support Professional to empower individuals with developmental disabilities, promote independence, and support daily living needs in our community programs.

You will implement and monitor care plans, assist with daily activities, document progress, accompany clients to appointments, and collaborate with healthcare professionals, families, and other staff to ensure holistic, person-centered support.

Qualifications

• High school diploma or GED required.
• Valid NY/NJ/CT/PA driver’s license with 6 months of experience and 6 or less points.
• Strong interpersonal and communication skills.
• CPR and First Aid certification preferred.
• Commitment to providing compassionate care and promoting dignity and respect.
• Ability to respond effectively to changing needs and environments.
• Maintain confidentiality, uphold ethical standards.

Responsibilities

• Implement and monitor the plans of care for persons supported, including plans such as Life Plan and Behavior Support Plans.
• Provide direct care and assistance with daily living activities, including personal hygiene and meal preparation.
• Assist with medication administration and accompanying individuals to appointments.
• Facilitate community outings and social activities to promote inclusion and independence.
• Document progress notes and incident reports; ensure regulatory compliance.
• Collaborate with interdisciplinary teams and families to develop comprehensive care plans.
• Advocate for rights and preferences, promote self-advocacy and safety.
