Qualifications

• High school diploma or equivalent.

Responsibilities

• Provide person-centered care based on individual goals.
• Assist with personal care tasks (grooming, bathing, dressing, meals).
• Offer emotional support and active listening.
• Engage clients in meaningful companionship and activities.
• Support medication management and document administration.
• Maintain safety and promptly report emergencies.
• Complete accurate client documentation and progress notes.
• Collaborate with healthcare teams, families, and staff.
• Pursue ongoing training and professional development.
