Job summary

MyPath in Connecticut is seeking caring professionals to provide direct care and supervision, implement ISPs/ITPs and BSPs, assist with activities of daily living, and promote independence and community engagement.

You will monitor progress, document health concerns, communicate with the team, and help ensure a safe, respectful environment in compliance with safety and regulatory requirements.

Responsibilities

• Provide direct care to individuals served, ensuring their health, safety, and well-being.
• Implement Individual Service Plans (ISPs) and Treatment Plans (ITPs) and Behavior Support Plans (BSPs).
• Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
• Facilitate activities that promote skill development, independence, recreation, and community engagement.
• Monitor and document progress, health concerns, and incidents in accordance with policy.
• Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
