Job summary

Qualifications

• High school diploma or equivalent.
• Valid driver's license with clean driving record and proof of auto insurance.
• Flexibility for varied shifts and coverage as needed.

Responsibilities

• Support clients with daily living activities and medication administration.
• Assist with mobility and transfers using a Hoyer lift.
• Help clients achieve personal goals like cooking and cleaning.
• Transport clients to appointments and community events.
• Follow and document progress on Person Centered Service Plans (PCSPs).
• Communicate regularly with supervisors and team members.
