Job summary

Hacienda HealthCare in Phoenix, AZ is seeking a Direct Service Professional to support residents in our group homes. This part-time, safety-sensitive role covers evening and weekend shifts, with options for additional hours as needed.

You will provide for residents' safety and well-being, assist with daily activities, transportation, and documentation, while adhering to company policies and training requirements such as CPR/First Aid and TB clearance.

Qualifications

• Minimum age 25 years for drivers.
• Clean driving record and approved by insurance carrier.
• Pass Hacienda Drivers Training.
• Flexible hours and locations.

• Pass and maintain Article 9 and Prevention and Support within 90 days.
• Provide MVD driving record by offer time.
• Pass CPR/1st Aid within 30 days.
• Obtain and maintain Class 1 Finger Print Clearance Card.

Responsibilities

• Resident Care: Provide for safety, emotional and physical needs; keep residents clean, well-groomed, and dressed; position and reposition hourly while awake and every two hours while asleep; implement and document ISP objectives; assist with meals and activities; provide stimulation.
• Driving: Daily vehicle maintenance checks; escort and drive residents to appointments; maintain clean driving record; report changes to driving record.
• Home Maintenance: Clean home and resident bedrooms; complete required documentation; follow safety standards and PPE; attend trainings; adjust for staff coverage.
• Company Compliance: Flexible hours/location; maintain required certifications (TB test, CPR & 1st Aid, etc.); keep license current; follow policies; document before shift end.

Shift Schedules

• Full‑Time: Monday - Thursday (11 pm to 9 am), Monday - Friday (3 pm to 11 pm)
• Part‑Time: Saturday and Sunday (9 am to 9 pm) Driver (Female), Thursdays (8 am to 8 pm)
• PRN (As needed)

Summary

Direct Service Professionals (DSPs) are responsible for performing essential care for our residents in our group homes under the direction of the Group Home Manager. This is a safety sensitive position.

Responsibilities

• Resident Care: Provide for the safety, emotional and physical needs of residents; ensure residents are clean, well‑groomed and dressed appropriately; position and reposition every hour while awake and every two hours while asleep; ensure ISP objectives are implemented and documented daily; help residents cook meals and eat; ensure residents have emotional and physical stimulation; assist with showering and laundry; pass medications safely.
• Driving: Daily vehicle maintenance checks; keep signed documents; escort and drive residents to appointments; maintain clean driving record; report changes to driving record immediately.
• Home Maintenance: Clean the home and resident bedrooms; complete required documentation (incident reports, HT daily duties) before shift end; follow safety standards and PPE; attend required training sessions; adjust for staff coverage as needed.
• Company Compliance: Flex hours and location as needed; maintain Article 9, Prevention and Support, TB skin test, Finger Print Clearance Card, CPR & 1st Aid, annual flu vaccine; keep drivers license current; adhere to company policy and directives; complete required documentation before shift end.
