Job summary

St. John’s Community Services seeks a Day Support Specialist to assist individuals with disabilities in developing daily living skills and participating in community activities. You will provide personal care support, documentation, and responsive supervision in training sites and during transportation.

  • Ability to model and teach relevant social and behavioral skills.
  • Ability to operate relevant computer systems, including hardware and software, and office machines.

Responsibilities

  • Carry out the mission and core values through relationship building and skills training via ISP implementation.
  • Facilitate active participation in community-based activities and contracted services.
  • Complete daily data collection and ISP documentation including behavioral supports.
  • Observe and report changes in individuals and coordinate follow-up.
  • Monitor and report unusual events per SJCS policy and state regulations.
  • Assist in developing activity calendars aligned with ISPs.
  • Provide safe transportation to activities and appointments.
  • Provide routine feedback on fleet transportation services.
  • Supervise individuals to ensure health and safety, including administering medications.
  • Attend agency events, trainings, and meetings.
  • Assist with medical appointments and other duties as assigned.

What are we looking for?

We are looking to add a Day Support Specialist to our team. As a Support Specialist you will participate as an active and contributing member of a dynamic and collaborative team. Provides direct support to individuals with disabilities in developing and/or maintaining skills that will enhance the quality of their lives in the community; assists with personal care and daily routines occurring in community training sites.

Your overall essential responsibilities will be the following:
  • Responsible for carrying out the mission and core values of the organization through relationship building activities internally and externally, as well as skills training acquired because of the implementation of the Individual Service Plan (ISP).
  • Facilitate individual’s active participation in community-based activities, including contracted services and volunteer activities.
  • Complete daily and routine data collection and documentation as stipulated in the Individual Service Plan (ISP) including behavioral support and other specialized plans
  • Observe, respond, and report/document individual’s physical, social, behavioral and personal needs by recognizing any changes in individuals that may require additional follow-up or attention
  • Monitor and report any unusual events in accordance with SJCS policy and state regulations.
  • Assist in the development of activity calendars to ensure that the activities conducted meet the needs of the individuals supported as indicated in ISPs.
  • Provide safe transportation to and from scheduled community activities, appointments, and/or community sites
  • Provide routine feedback regarding fleet transportation services as needed and requested.
  • Provides adequate supervision to individuals to facilitate their health and safety including administering medication that is timely and in the correct dosage
  • Attends and participates in all relevant agency events, workshops, training and meetings.
Secondary Functions:
  • Assist with medical appointments.
  • Performs other job-related duties as assigned.

QualificationsRequired Knowledge, Skills, and Abilities:
  • Ability to gain working knowledge of St. John’s Community Services policies and procedures.

You will catch our eye if you have the following knowledge, skills, and abilities:
  • High school diploma or G.E.D. preferred.
  • Two or more years’ experience working with people with disabilities preferred.
  • Experience in community-based day programs a plus.
All SJCS employees must:
  • Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
  • Meet SJCS and the funding authority requirement of criminal background and record checks.
  • Valid state-issued driver's license with current driver's record in good standing
