Job Title: Direct Support Professional (DSP)
Job Summary:

Prestige Home Care LLC is seeking a compassionate and dedicated Direct Support Professional (DSP) to provide person-centered support to individuals with intellectual and developmental disabilities (IDD) in their homes and communities. As a DSP, you will assist individuals in developing and maintaining independence, participating in meaningful activities, building relationships, and achieving their individual goals as identified in the Individual Support Plan (ISP).

Essential Duties and Responsibilities:
  • Provide individualized, person-centered support based on the individual's ISP and service needs.
  • Assist individuals with activities of daily living (ADLs), instrumental activities of daily living (IADLs), and other approved support needs.
  • Support individuals with personal hygiene, grooming, dressing, meal preparation, housekeeping, and other daily activities as authorized.
  • Encourage independence by allowing individuals to make choices and participate in their own care and daily routines.
  • Support individuals in developing and maintaining life skills, including communication, self-advocacy, decision-making, money management, and household skills.
  • Assist individuals with accessing community resources, recreational activities, appointments, employment, education, volunteering, and social activities as identified in the ISP.
  • Promote meaningful community integration and participation.
  • Provide transportation assistance when authorized and consistent with the individual's service plan and agency policy.
  • Maintain a safe environment and identify and report safety concerns.
Person-Centered Practices:
  • Promote choice, control, dignity, and independence.
  • Support individuals to exercise their rights and make informed decisions.
  • Focus on the individual's strengths, preferences, abilities, and goals.
  • Encourage opportunities for meaningful relationships and community participation.
  • Avoid unnecessary restrictions and promote the least restrictive approach consistent with safety.
  • Provide support without discrimination, intimidation, coercion, or retaliation.
Health and Safety Responsibilities:
  • Follow all agency health and safety policies.
  • Respond appropriately to emergencies and immediately notify the appropriate supervisor or emergency services when necessary.
  • Follow infection prevention and control procedures.
  • Follow approved medication-related procedures when authorized and trained to perform such tasks.
  • Report accidents, injuries, incidents, suspected abuse, neglect, exploitation, and significant changes in condition according to required timelines and agency policy.
  • Maintain a safe and clean environment for the individual.
Documentation and Compliance:
  • Complete required documentation accurately, objectively, and within established timelines.
  • Documentation may include daily service notes, progress notes, EVV/timekeeping records, incident reports, communication logs, individual progress documentation, and other records required by Prestige Home Care LLC, the individual's ISP, or applicable Pennsylvania requirements.
  • Falsification of records, inaccurate time reporting, failure to document services, or providing services outside the authorized scope may result in corrective action, up to and including termination.
Qualifications:
  • High school diploma/GED preferred; applicable requirements will be followed for the specific ODP service.
  • Ability to communicate effectively with individuals, families, and the support team.
  • Ability to follow written and verbal instructions.
  • Ability to maintain confidentiality and professional boundaries.
  • Ability to complete required documentation.
  • Ability to work independently and as part of a team.
  • Successful completion of required background checks, clearances, and agency screening requirements.
  • Successful completion of required DSP orientation, training, and competency requirements.
Physical Requirements:
  • Stand, walk, bend, reach, and sit for extended periods.
  • Assist individuals with mobility and daily activities within the scope of the authorized service and the employee's training.
  • Perform light housekeeping and meal preparation as required by the individual's service plan.
  • Respond appropriately to emergency situations.
