• Communicate appointment outcomes, provider updates, and relevant health information to appropriate team members.
• Maintain transportation logs, medical visit records, and incident reports.
• Assist with direct support services as needed, including supervision, coaching, and support with daily living or community integration activities.
• Maintain vehicles in safe operating condition, including inspections, cleanliness, and reporting maintenance needs.
• Follow company policies, safety procedures, licensing requirements, and mandated reporting obligations while collaborating with team members to support coordinated care.
