Job summary

Role includes aiding with daily living tasks, documenting visits accurately, and assisting with care plans. Team collaboration ensures coordinated care and adherence to policies and reporting obligations.

**Overview****Essential Duties*** Provide safe transportation for individuals served to medical appointments, community activities, programs, errands, and other destinations.* Supervise and support individuals served during transportation, appointments, and community activities while ensuring their safety and well-being.* Support implementation of Individual Treatment Plans (ITPs), Behavior Treatment Plans (BTPs), and other care plans.* Communicate appointment outcomes, provider updates, and relevant health information to appropriate team members.* Maintain accurate and timely documentation including transportation logs, medical visit records, and incident reports.* Assist with direct support services as needed, including supervision, coaching, and support with daily living or community integration activities.* Maintain vehicles in safe operating condition, including inspections, cleanliness, and reporting maintenance needs.* Follow company policies, safety procedures, licensing requirements, and mandated reporting obligations while collaborating with team members to support coordinated care.**Additional Information**MyPath and its Operating Companies provide specialized services and dignified care for children, adolescents and adults with special needs.
