Job summary

Sevita is seeking a Direct Support Professional to provide hands-on care and support to individuals in our community-based programs in Riverside, California. You will work as part of a dedicated team to implement individual service plans, assist with daily living activities, and promote independence and well-being.

The role is hourly, non-exempt, with shift-based scheduling. You will document progress, ensure safety, transport as needed, and collaborate with nurses and supervisors to deliver

Qualifications

• Six months of experience in human services preferred.
• High school diploma or equivalent preferred.

Responsibilities

• Provide direct support or assistance according to individual plans.
• Assist with daily living activities, transportation, and community integration.
• Document progress and activities accurately and maintain confidentiality.
• Support health and safety, follow care plans, and report changes.
• Collaborate with clinical staff and supervisors to deliver quality care.
