Job summary

Hope Network in Michigan is seeking Direct Support Professionals (DSP) who are committed to helping individuals build independence and meaningful community connections.

The DSP will support daily living activities, monitor safety, assist with health needs, accompany to appointments, and help individuals participate in social and vocational activities, all while promoting dignity, respect, and personal choice.

Qualifications

• Commitment to scheduled shifts including weekends and holidays.

Responsibilities

• Monitor and ensure safety 24/7 and assist with daily living activities.
• Support crisis prevention, intervention, and resolution.
• Establish collaborative relationships built on trust and empathy with individuals served and the care team.
• Foster partnership with individuals, the community, and other professionals.
• Assist with social, community, and vocational activities as requested.
• Attend medical appointments and relay information accurately to the care team.
• Document care plans and progress clearly and timely.
