Job summary

Heart to Heart Health Care Services is seeking compassionate Direct Support Professionals to work with individuals with Intellectual and Developmental Disabilities in a group home environment. You will provide daily living support and transportation, helping residents maintain dignity and independence.

Roles include assisting with personal care, promoting safety and participation, and supporting activities that foster self-care and social skills.

Qualifications

• Driver’s license and second form of ID.
• Physical and PPD within the last year.
• Diploma or equivalent.

Responsibilities

• Direct support to individuals with IDD in a group home setting.
• Provide support, transportation and assistance to enable daily routines.
• Assistance with toileting, bathing, dressing, laundry and supervision of recreation.
• Encourage independence through skill training in self-care, household chores and social/recreation activities.

Job Description

Direct Support to individuals with Intellectual and Developmental Disabilities in a Group Home setting. Provide support, transportation and assistance for each individual to enable her/him to complete daily routines in a safe, healthy, productive and dignified manner. This usually includes toileting, bathing, dressing, laundry, supervision of recreation. Encourages independence by providing both the opportunity and the appropriate levels of skill training for the individuals in categories such as self-care, household chores, social skills, recreation skills, etc.
