Job summary

Creative Connections, LLC is seeking a Direct Support Professional to support individuals in their own homes across Southwest Ohio. The role requires a valid driver’s license, reliable vehicle, and ability to lift up to 50 lbs.

Starting wage is $18.00 per hour for awake hours with raises over time; OSOC hours are paid at $13.50 per hour. Full-time employees may access medical, dental, vision, 401(k), PTO and holidays.

• Safe and reliable vehicle for shift hours to transport individuals.

Responsibilities

• Provide a variety of care and support services to individuals living in their own homes.
• Assist with household tasks such as laundry and cleaning.
• Assist with bathing and personal hygiene tasks; medication administration as directed.
• Plan and prepare meals; assist with eating.
• Transport individuals to medical appointments and community events.

Job description

Applicants must be at least 18 years old at the time of application, have at least a high school diploma (or equivalent), have a valid driver's license with an acceptable driving record, have a safe and reliable vehicle available to use during shift hours to transport individuals receiving services, and have at least state-minimum automobile insurance.

Applicants must also be able to safely lift up to 50 lbs. unassisted and be able to carry, push, pull, bend, transfer, stoop and walk safely without assistance.

If applicable, OSOC (sleep) hours are paid at a rate of $13.50 per hour while the individual supported is asleep.

Compensation

• The starting wage for this position is $18.00 per hour for regular (awake) hours with regular increases.
• OSOC (sleep) hours are paid at a rate of $13.50 per hour while the individual supported is asleep.
• Employees will receive double-time pay, up to sixteen (16) hours, for shifts worked on the Company's seven (7) premium holidays, and time and a half, up to sixteen (16) hours for shifts worked on the Company's five (5) standard holidays.
• Full-time employees are eligible to participate in the Company's benefit plans, including medical, dental and vision insurance, 401(k) retirement plan with Company match, paid time off, and paid holidays.

Requirements

Direct Support Professionals provide a variety of care and support services to the individuals we support who live in their own homes throughout Southwest Ohio.

Some of the services you may provide include assistance with household tasks, including laundry and cleaning, bathing and person hygiene care, medical care tasks including medication administration, meal planning and preparation, assistance with eating, and transporting the person to and from medical appointments and community events.
