Job summary

Heart to Heart Home is seeking committed Direct Support Professionals to support individuals with Intellectual and Developmental Disabilities in our Clayton and Williamstown group homes. This role involves compassionate direct care, helping residents with daily activities and transportation to enable safety and dignity.

The position supports residents in activities of daily living, promotes independence through skill-building, and requires a valid driver’s license and basic health checks.

Qualifications

• Driver’s License and second form of ID
• Physical and PPD within the last year
• Diploma or Equivalent

Responsibilities

• Provide direct support to individuals with IDD in a group home setting.
• Assist with transportation and daily routines to promote independence.
• Assist with toileting, bathing, dressing, laundry, and supervision of recreation.
• Encourage independence through skill training in self-care, household chores, social and recreation skills.

Job Description

Direct Support to individuals with Intellectual and Developmental Disabilities in a Group Home setting. Provide support, transportation and assistance for each individual to enable her/him to complete daily routines in a safe, healthy, productive and dignified manner. This usually includes toileting, bathing, dressing, laundry, supervision of recreation. Encourages independence by providing both the opportunity and the appropriate levels of skill training for the individuals in categories such as self-care, household chores, social skills, recreation skills, etc.
