Job summary

The Summit Center in Amherst, NY is seeking a Direct Support Professional to work in a community-based day program, helping adults with developmental disabilities participate in recreation and life skills training. You will promote independence and collaborate with a dedicated team to support individuals in reaching their goals.

Responsibilities include documenting services, communicating needs to the Family Support Services Coordinator, and attending staff meetings.

Qualifications

• High school diploma or equivalent.
• 1 year of professional experience working with individuals with developmental disabilities.
• Dayhab experience preferred.

Responsibilities

• Support adults with developmental disabilities in a community-based day program.
• Provide recreation, life skills training and promote independence.
• Complete and maintain accurate documentation of services and progress.
• Communicate individual needs to Family Support Services Coordinator.
• Attend staff meetings and in-service training programs.
• Maintain confidentiality per NYS Mental Hygiene law and HIPAA.
• Be flexible to accommodate recreation outings and activities.
• Perform other duties as requested by supervisor.

Education

High School Diploma or equivalent

1 year of professional experience with individuals with developmental disabilities

Schedule: Monday - Friday, 8:00 AM - 4:00 PM, 40 hours/week

Your Impact As a Direct Support Professional:

The Direct Support Professional supports adults with developmental disabilities in a community-based day program, providing recreation, life skills training, and promoting independence. This role offers a collaborative team environment and active community engagement.

• Follows through on any behavior management program as established for individuals.
• Completes and maintains accurate documentation relating to the various services provided to our individuals and the progress they have established.
• Communicates individual needs to Family Support Services Family Support Coordinator.
• Must be able to work flexible hours, on occasion to accommodate recreation outing and activities.
• Attend staff meetings and in-service training programs.
• Complies with agency incident reporting policies and procedures.
• Maintains confidentiality of individual information in accordance with the requirements of NYS Mental Hygiene law and the Agency’s HIPAA Privacy Policies and Procedures
• Perform any other duties as requested by your supervisor.

EDUCATION And/or EXPERIENCE:

• High school diploma or equivalent
• 1 year of professional experience working with individuals with developmental disabilities
• Dayhab experience preferred

CERTIFICATES, LICENSES, REGISTRATIONS:

A valid NYS Driver’s License and agency approval of driving record according to agency’s Policy and Procedures
