Qualifications

  • Must possess a NYS driver's license with an acceptable driving record.

Responsibilities

  • Facilitate active participation of residents in habilitation plans in family homes, respite and community.
  • Assist with lifting, moving and positioning tasks as needed.
  • Support gross, fine motor and range of motion skills.
  • Promote self-care, activities of daily living and personal hygiene.
  • Foster communication through sign language, pictograms, and devices.
  • Assist with recreational activities and community outings.
  • Implement SCIP-r behavior support plans accurately.
  • Complete documentation using PrecisionCare software and written forms when necessary.
  • Provide personal and physical care including bathing, toileting, dressing and eating as needed.
  • Perform housekeeping tasks as needed.
  • Follow HIPAA confidentiality laws and agency ethics.
  • Attend staff meetings and participate in required trainings.

Duties and Responsibilities

  • Facilitate active participation of the residents by implementing their habilitation plans in their family home, respite and/or community
  • Engage in any necessary lifting, moving and/or positioning tasks for individuals
  • Facilitate improved gross motor, fine motor and range of motion skills
  • Promote and help to facilitate self-care, activities of daily living and personal hygiene
  • Foster receptive and expressive communication through many different avenues including sign language, pictograms and electronic devices
  • Assist with recreational activities and facilitate active engagement in community outings
  • Implement behavior support plans and Strategies for Crisis Intervention and Prevention (SCIP-r) accurately and consistently
  • Accurately and consistently complete all required documentation in a continuous manner by utilizing PrecisionCare software and in written form, when necessary
  • Facilitate and provide for the personal and physical care needed by each individual including bathing, toileting, dressingand eating as needed including modeling appropriate behavior, using therapeutic equipment and/or therapeutic feeding
  • Perform housekeeping tasks when needed (meal preparation, cleaning, laundry, shopping, etc.)
  • Follow HIPAA confidentiality laws
  • Adhere to the Code of Ethics for DSPs
  • Attend and actively participate in regular staff meetings, case reviews, in-service sessions and any other meetings as necessary
  • Implement fire, safety and emergency procedures in accordance with agency and program standards
  • Maintain certifications in Medication Administration, SCIP-r, CPR and First Aid
  • All other duties as assigned

Minimum Qualifications

  • Must be at least 18 years of age
  • Ability to read, write, speak and understand English including interpreting documents such as safety rules and policy and procedure manuals
  • Ability to participate in various physical and recreation activities outside the residence
  • Possess a current and valid NYS driver’s license with an acceptable driving record

Preferred Qualifications

  • High School Diploma or equivalent
  • 6 months or more experience in similar role

Certifications and Trainings

  • SCIP-r
  • CPR
  • First Aid
  • All other trainings mandated by the Office for people with Developmental Disabilities (OPWDD)

*Note: all certifications and trainings will be provided by the agency if you do not already possess them
