Job summary

Holcomb Behavioral Health Systems is seeking staff to provide support and personal assistance to residents in a community group home under the supervision of the Division Manager of Residential Services. Responsibilities include implementing individual plans, modeling positive behavior, and coordinating transportation for appointments and activities.

Education required is a high school diploma or equivalent; no prior experience is necessary.

Qualifications

• High school diploma or equivalent.
• Experience: None required.
• Licensure/Certifications: None Required.
• Required Clearances: Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list.

Responsibilities

• Assist persons served in reaching their goals of becoming more independent.
• Act as a positive role model for persons served.
• Provide guidance, instruction, coaching, and support, in accordance with individual plans.
• Engage individuals in beneficial programs and activities.
• Transport and accompany individuals into the community for appointments and outings.
• Directly assist with personal care as needed.
• Assist with meal preparation and routine homemaking duties.
• Assume responsibility for the safety of the person served including medical/medication needs.
• Apply approved behavior plans and intervention strategies as/if needed.
• Complete records and reports; collect data according to Agency policy.
