Job summary

Lifetime Assistance, Inc. in Rochester, NY is seeking a Direct Support Professional – Per Diem I to provide respectful personal care and support individual goals in day and residential settings.

This on-site, part-time role requires a per diem schedule with at least 16 hours every two weeks and starting wages of $17.44/hr (Day Services) or $18.44/hr (Residential). You will drive to appointments as needed, follow individualized plans, document in the EHR, and work with a mission‑driven team to

Qualifications

• Maintain SCIP-R and First Aid/CPR certifications.
• Pass required background checks and clearances (SCR, fingerprinting, SEL, MHL).
• Able to lift/move up to 35 pounds and perform physical care tasks.
• Preferred: experience in human services or direct care.

Responsibilities

• Provide respectful personal care including bathing, grooming, toileting, dressing, and meal support.
• Support individual goals, independence, and skill development while promoting community inclusion.
• Implement individualized plans including IPOP, SAP, BSP, and PONS.
• Maintain accurate documentation in the EHR.
• Ensure health, safety, dignity, and well‑being; report concerns.
• Foster positive communication with individuals, families, co‑workers, and supervisors.
• Drive agency vehicles to transport individuals to appointments, programs, or community outings.
