Job summary

A home health care service provider is seeking a Direct Support Professional in Philadelphia to provide compassionate, person-centered care to individuals in various environments. Responsibilities include supporting daily living activities, facilitating therapeutic activities, and maintaining documentation of progress. The ideal candidate will have a High School Diploma or GED, with a preference for two years of experience working with individuals with developmental disabilities. Flexible scheduling and a supportive work environment are offered.

Qualifications

• Must be trained on ISPs of all individuals supported.
• Completion of 24 hours of agency-mandated annual training.
• Two (2) years of experience working with individuals with developmental/intellectual disabilities preferred.

Responsibilities

• Support individuals in achieving goals as outlined in their Person-Centered Plan.
• Provide personal care, instruction, guidance, and companionship.
• Assist with activities of daily living such as feeding, bathing, and dressing.
• Document progress toward outcomes in daily progress reports.
• Ensure the health and safety of individuals served.
