Job summary

Beyond Home Health Care in New Jersey is seeking a Direct Support Professional (DSP) to assist clients with daily living activities, implement ISP plans, and promote independence in the community.

You will provide compassionate care, monitor health, assist with transportation, and document care while supporting clients to achieve personal goals in a respectful, client-centered environment.

Qualifications

• High School Diploma or GED required.

Responsibilities

• Assist with daily living activities including hygiene, meals, and mobility.
• Implement ISP plans aligned with client goals.
• Provide companionship and support to encourage social inclusion.
• Administer medication and monitor health as directed.
• Assist with transportation to appointments and community activities.
• Maintain accurate care documentation and follow behavior support plans.
