New Horizons in Watertown, SD is seeking a Direct Support Coordinator to lead and empower individuals supported by our agency. You will develop and monitor individualized support plans, supervise Direct Support Professionals, and ensure health, safety, rights, and well-being while collaborating with families and professionals.

Requirements include a High School Diploma or GED, 2 years' experience with people with disabilities, and preferred supervisory experience.

Qualifications

• High School Diploma or GED required.
• Two years' experience working with people with Disabilities preferred.
• Supervisory experience preferred.

Responsibilities

• Support people in achieving their goals, preferences, and desired outcomes.
• Advocate for individuals and ensure their rights and needs are respected.
• Participate in developing and monitoring Individual Support Plans.
• Provide leadership, supervision, and support to Direct Support Professionals.
• Ensure staff follow individual plans, agency policies, and state regulations.
• Monitor staffing, schedules, training, documentation, and service quality.
• Communicate and collaborate with individuals, families, guardians, professionals, and team members.
• Respond to incidents or changes in individual needs and coordinate additional support when needed.
• Maintain accurate records and ensure required documentation and assessments are completed on time.
• Help ensure a safe, clean, and supportive living environment.
