Join Our Team as a Direct Home Care Worker at PACE Southeast Michigan

We are seeking dedicated individuals to provide supportive services to participants in home, community, and center settings. Under the supervision of the Center Manager or Team Lead/Home Care Coordinator, the Direct Home Care Worker plays a vital role in enhancing the well-being and independence of our participants.

Responsibilities include:

• Implementing care plans based on assessments by RN and care team.
• Providing physical and emotional support to participants.
• Assisting with activities of daily living, including personal care, meal prep, toileting, and medication reminders.
• Supporting transportation and off-site appointments.
• Accurate documentation of services provided.
• Reporting any issues or emergencies promptly.

Qualifications:

• High School Diploma or G.E.D. required.
• At least 2 years of relevant experience or completion of formal training (e.g., Nurse Assistant, Medical Assistant).
• Participation in annual training including infection control, TB testing, safety, and BLS.
• Ability to work with diverse populations and handle interruptions.
• Valid driver’s license and reliable transportation.

Working Conditions:

• Driving within the catchment area is required.
• Exposure to communicable diseases and chemicals is possible.
