Position Overview

Under the direction of the Center Manager and/or Team Lead/Home Care Coordinator, the Direct Home Care Worker provides supportive services to participants in various settings, including in-home, community, transportation, appointments, and day health centers. Responsibilities are outlined in the participant's Care Plan and include a range of supportive tasks.

Specific Duties and Functions
  1. Develop and follow the plan of care based on assessments by a home care RNCM.
  2. Provide physical and emotional comfort to frail elders in various settings.
  3. Assist participants and families to achieve independence.
  4. Perform tasks such as personal care, activity assistance, meal prep, toileting, simple treatments, shopping, medication reminders, laundry, homemaking, and transportation.
  5. Accurately document services provided and communicate progress to supervisors.
  6. Report customer service issues and schedule deviations promptly.
  7. Respond to emergencies per policies and procedures.
  8. Work within the scope of practice.
Qualifications
  • High School Diploma or G.E.D. required.
  • At least 2 years of relevant work experience or completion of formal training (e.g., Nurse Assistant, Medical Assistant).
  • Participation in annual training including infection control, TB testing, safety, and BLS.
  • Ability to establish relationships, respect participant diversity, and work with individuals with dementia or disabilities.
  • Must meet CMS competencies and be medically cleared with up-to-date immunizations.
Working Conditions
  • Driving within the catchment area required; reliable transportation and valid driver’s license needed.
  • Exposure to communicable diseases and chemicals possible.
