A Place Called Home LLC is seeking Direct Support Professionals to provide Individualized Home Supports (IHS) with Training in Minneapolis and St Paul. You will teach daily living skills and support independence through hands-on coaching in clients’ homes and community settings.

Ideal candidates are reliable, patient, and motivated to make a meaningful impact. You will follow CSSP care plans, document progress, and use EVV to track services while maintaining professional boundaries and client

Competenze

• Experience working with individuals with disabilities preferred.
• Strong communication and documentation skills required.
• Reliable transportation and ability to pass background checks.
• Ability to work independently and follow care plans.

Job Description (Post This):

We are currently hiring Direct Support Professionals (DSPs) to provide Individualized Home Supports (IHS) with Training services to clients in Minneapolis and St Paul.

This role focuses on teaching and supporting individuals to build independence in their daily lives. We are looking for dependable, patient, and motivated staff who are passionate about making a meaningful impact.

Responsibilities:

• Provide IHS with Training services in clients’ homes and community
• Support and teach independent living skills (cooking, cleaning, budgeting, community integration, etc.)
• Follow each client’s Coordinated Service and Support Plan (CSSP)
• Document daily progress notes tied to client goals
• Use EVV (Electronic Visit Verification) to track time and services
• Maintain professional boundaries and ensure client safety at all times

Locations:

Minneapolis and St Paul

Hours:

• Up to 40 hours per week available
• Flexible scheduling based on client needs

Requirements:

• Experience working with individuals with disabilities preferred
• Strong communication and documentation skills
• Reliable transportation (required)
• Must pass background study
• Ability to work independently and follow care plans
