Job summary

JEVS Human Services Career Center in Philadelphia is hiring Direct Care Workers for the Community Living & Home Supports (CLHS) team. This floater position means you’ll bring your care to different homes and locations, with weekend shifts.

Starting pay is $19.09 per hour for floaters, with guaranteed part-time hours (30 per week) and paid training. You’ll support adults with disabilities, helping with daily living, community activities, and health and safety.

Responsibilities

• Provide person-centered support to adults with disabilities, promoting dignity, choice, independence.
• Assist with daily living and personal care (meals, bathing, dressing, toileting, housekeeping, medications) per plan.
• Support community involvement and skill development by accompanying individuals to appointments and activities.
• Maintain a safe, welcoming home environment while following service plans and safety procedures.
• Document and communicate changes in health or behavior with supervisors and the team.

What You’ll Do

• Provide person-centered support to adults with intellectual, physical, and/or behavioral disabilities, promoting dignity, choice, independence, and self-determination.
• Assist with daily living and personal care, including meals, bathing, dressing, toileting, household tasks, medication administration, and other needs based on the individual’s support plan.
• Support community involvement and skill development by accompanying individuals to appointments, recreational activities, volunteer opportunities, and other community experiences.
• Maintain a safe, supportive, and welcoming home environment, while implementing individualized service plans, behavioral strategies, and health and safety procedures.
• Document and communicate effectively by completing required records, monitoring and reporting changes in individuals’ health or behavior, and working collaboratively with supervisors and the individual’s support team.
