Job summary

Harboroaks seeks a compassionate facilitator to provide one-on-one or group instruction/assistance to patients on a daily basis. You will help with program activities, monitor data, assist with meals, and promote independence in activities while maintaining a clean, therapeutic environment.

Must have a High School Diploma or equivalent and be at least 21 years old with a valid driver’s license. The role emphasizes patient safety and a professional demeanor.

Qualifications

• High School Diploma or equivalent is required.
• Must be at least 21 years of age.

Responsibilities

• Provide one-on-one or group instruction/assistance to patients daily.
• Complete routine daily checks of van (if driving) and report deficiencies to supervisor.
• Accurately monitor and maintain data collection sheets and submit to supervisor on time.
• Assist with assessment of clients' strengths and needs as necessary.
• Assist with preparation of clients' meals and provide client training as needed.
• Communicate with supervisor to revise, delete, or add training objectives as needed.
• Physically assist in maintaining a neat and orderly facility.
• Monitor and document individual behaviors with descriptive terms.
• Encourage independence and participation in activities.
• Provide positive assurance to individuals regarding benefits of treatment and participation.
• Promote a therapeutic environment that supports learning new skills.
