Job summary

CONFIDENTIAL is seeking a Direct Care PRP Counselor to support youth and adults with mental illness. The role focuses on enhancing quality of life through patient, compassionate care and consistent program involvement.

The ideal candidate will participate in individual and group sessions, collaborate with clinical staff, and assist with rehabilitation activities while maintaining detailed records and family communication.

Qualifications

• 2 years experience working with mentally ill children or adults.
• Complete & pass a federal CJIS background check.
• Valid driver’s license and reliable transportation.
• Ability to adhere to all health and safety guidelines.
• Excellent communication and interpersonal skills.
• Ability to maintain quality records and timely documentation.

Responsibilities

• Meet with clients for individual and group sessions.
• Participate in treatment planning and communicate with clinical staff, Program Director, and clients' families.
• Attend and complete Ascend's required training.
• Maintain contact with the client’s family and treatment team.
• Implementation of rehabilitation activities.
• Assist clients in achieving their individual goals by teaching and supporting them while linking them to community resources.
• Maintain quality records and timely documentation.
