Job Summary
We are seeking a Direct Care PRP Counselor to join our team and support youth and adults with mental illness in enhancing their quality of life. As a PRP Counselor, you will provide essential services to help clients achieve their rehabilitation goals, including assistance with daily living skills, personal hygiene, organization, building supportive networks, and promoting a positive and supportive living environment. The ideal candidate is patient, compassionate, reliable, and committed to fostering independence and empowerment for individuals in need.
Responsibilities
- Meet with clients for individual and group sessions.
- Participate in all treatment planning, communicate consistently with clinical staff, Program Director, and clients' families.
- Attend and complete Ascend's required training.
- Maintain contact with the client’s family and treatment team.
- Implement rehabilitation activities.
- Assist clients in achieving their individual goals by teaching and supporting them while linking them to various community resources.
- Maintain quality records and timely documentation.
Qualifications
- At least 2 years of experience working with mentally ill children or adults.
- Complete & pass a federal CJIS background check.
- Valid driver’s license and reliable transportation.
- Ability to adhere to all health and safety guidelines.
- Excellent communication and interpersonal skills.
- Ability to maintain quality records and timely documentation.
- Compassionate, respectful, and ethical attitude.
