Qualifications

• Experience as a caregiver / direct care provider.
• Ability to independently manage scheduling and time.
• Basic proficiency with digital documentation or monitoring tools.
• Three (3) professional references.
• Valid Fingerprint Clearance Card (or ability to obtain).
• Successful completion of required background checks.
• Experience as a Direct Care Worker that has completed DCW Training.
• Bilingual proficiency in English/Spanish is a plus.

Responsibilities

• Conduct four required monitoring reviews per member per provider each year: 2 in-person visits in the member's home; 2 virtual visits using a secure video platform.
