Job summary

Indiana University Health is seeking a nutrition counselor to provide telephonic education and disease-state guidance to team members enrolled in Healthy Results. The role emphasizes evidence-based medical nutrition counseling, care planning, and risk stratification within a hybrid work setting.

You will educate, assess, and follow up with participants, coordinating referrals and updates to patient education materials while collaborating with health coaches and clinicians.

Qualifications

• Master's Degree is required or Bachelor's Degree prior to 2024.
• Requires 3-5 years of relevant experience.
• Registration through the Commission on Dietetic Registration within 6 months.

Responsibilities

• Educate patients on disease risk factors, prevention, and medical nutrition counseling.
• Assess and follow up with participants in the dietitian program; identify gaps and refer as needed.
• Conduct outreach with risk-stratified individuals and collaborate with health care professionals.
