Job summary

Indiana University Health is seeking a nutrition-focused dietitian to provide telephonic counseling and education to team members enrolled in Healthy Results. The role involves conducting one-on-one and group educational sessions, following established guidelines, and collaborating across health care teams.

You will develop individualized nutrition care plans, perform nutrition-focused assessments, and support patients with chronic disease management, including diabetes, with limited travel.

Qualifications

• Master's degree or Bachelor's degree prior to 2024 is required.
• 3-5 years of relevant experience.
• Registration through the Commission on Dietetic Registration within 6 months of start date.
• Licensure through the state of Indiana within 1 year of start date.
• Ability to develop nutritional care plans and perform nutrition-focused physical exams.

Responsibilities

• Provide telephonic nutrition counseling and education to IU Health team members enrolled in Healthy Results.
• Assess, implement, evaluate, and follow up on participants in the dietitian program.
• Conduct proactive outreach and intervene with risk stratified individuals.
• Serve as a resource to patients and health care professionals within Healthy Results.
