Job summary

Elderwood at Waverly is seeking a Registered Dietitian (Full‑time) to support residents in long‑term care and short‑term rehab. The CDN license or NY licensure is required, with active CODR registration preferred; long‑term care experience is a plus.

The role includes nutrition assessments, care plan development, counseling, and coordination with an interdisciplinary team. Join a team focused on compassionate resident care and regulatory compliance.

Qualifications

• Bachelor of Science in Nutrition/Dietetics from an accredited program.
• Successful completion of an internship or equivalent qualifying for registration.
• Active and ongoing Registration with the Commission on Dietetic Registration.
• CDN licensure with NYS Ed or licensure in state of employment (or within 1 year).
• Long term care experience preferred.
• Computer skills and knowledge in nutritional software programs preferred.
• Must read/write/speak English at intermediate level or higher.

Responsibilities

• Completes nutrition assessments for adult/geriatric residents in LT care and sub‑acute rehab.
• Develops and documents individualized nutrition care plans based on best practices.
• Provides nutrition counseling/education for residents and families.
• Maintains resident nutrition information in the computer system.
• Conducts routine audits and monitors quality improvement initiatives related to nutrition and food service.
• Attends resident care plan meetings and required in‑service programs.
• Maintains knowledge of federal and state nutrition regulations for long‑term care.
• Serves as nutrition consultant to the Administrator and multidisciplinary team.
• Acts as a preceptor for dietetic students and a supervisor/mentor to Diet Technicians.
• Conducts in‑service programs for dietary and facility staff on request.
• Supports and abides by Elderwood's Mission, Vision and Values.
• Abides by code of conduct and HIPAA policies.

Registered Dietitian (Full-time) – supports residents in long‑term care and short‑term rehab.

Salary: Up to $75,000 per year.

Responsibilities

• Completes nutrition assessments for the adult/geriatric population in a long‑term care and sub‑acute rehabilitation setting in accordance with Elderwood policies and standards of care.
• Develops, implements, monitors, and appropriately documents individualized nutrition care plans based on current best practices.
• Provides nutrition counseling/education for residents and their family members.
• Maintains resident nutrition information in the computer system.
• Conducts routine audits and monitors performance improvement initiatives related to nutrition and food service.
• Attends and actively participates in resident care plan meetings, staff meetings, required in‑service programs, and other meetings/programs deemed appropriate by the Administrator or Director of Nutritional Services.
• Maintains a working knowledge of federal and state regulations regarding nutritional services in the long‑term care setting.
• Serves as a nutrition consultant for the Administrator, Diet Technician, Food Service Director, Dietary Supervisor, and multidisciplinary medical team.
• Acts as a preceptor for dietetic students and interns and as a supervisor/mentor to the Diet Technician(s).
• Conducts in‑service programs for dietary and facility staff upon request.
• Assists the Food Service Director and/or Dietary Supervisor with department oversight on an as‑needed basis.

• Adheres to the Academy of Nutrition and Dietetics Code of Conduct and Professional Standards.

Qualifications

• Bachelor of Science Degree or higher in Nutrition/Dietetics from an accredited educational program.
• Successful completion of an internship or equivalent clinical management practice which would qualify for Professional Registration with the Commission on Dietetic Registration.
• Active and ongoing Registration with the Commission on Dietetic Registration.
• Active membership in The Academy of Nutrition and Dietetics preferred.
• Certified Dietitian - Nutritionist (CDN) with The New York State Department of Education or Licensure in the state of employment (or to be obtained within one year from date of hire).
• Long term care experience preferred.
• Computer skills and knowledge in nutritional software programs preferred.
• Requires regular interaction with residents, coworkers, visitors, and/or supervisors. Must be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
