Job summary

Elderwood at Waverly in Liverpool, NY seeks a Registered Dietitian (license required) for a full-time role supporting residents in long-term care and short-term rehab. You will conduct nutrition assessments, develop care plans, educate residents and families, and maintain detailed documentation in the EMR.

Qualifications

• Bachelor’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian credential required.
• Active registration with the Commission on Dietetic Registration (CDR).
• Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable.
• Long-term care, rehabilitation, or geriatric experience preferred.
• Strong communication and interdisciplinary collaboration skills.
• Experience with nutritional software and electronic medical records preferred.

Responsibilities

• Complete nutrition assessments and reassessments for residents.

• Monitor residents’ nutritional status, weight trends, intake, and overall wellness.
• Maintain accurate nutrition documentation within the electronic medical record system.

• Partner with Dining Services to support appropriate meal planning and service.
• Serve as a nutrition resource for facility leadership and clinical teams.
• Support and mentor Diet Technicians and dietetic interns, as applicable.
• Conduct nutrition-related audits and performance improvement initiatives.
• Maintain compliance with CMS, DOH, and nutritional service regulations.
• Assist with staff education and in-service training programs.
• Support survey readiness and best practice initiatives.

Responsibilities

Registered Dietitian (RD / CDN):
