Job summary

Elderwood at Waverly in Liverpool, NY is seeking a Registered Dietitian (RD) / CDN for a full-time role to support residents in long-term care and rehab. You will perform nutrition assessments and develop care plans to improve resident health and quality of life.

Responsibilities include education for residents and families, monitoring status, and collaborating with Dining Services and interdisciplinary teams in a resident-centered environment. Requires CDN and NY licensure as applicable.

Qualifications

• Bachelor’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian credential required.
• Active registration with the Commission on Dietetic Registration (CDR).
• CDN or state licensure as applicable.
• Long-term care, rehabilitation, or geriatric experience preferred.
• Strong communication and interdisciplinary collaboration skills.
• Experience with nutritional software and electronic medical records preferred.

• Complete nutrition assessments and reassessments for residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition education and counseling to residents and families.
• Monitor residents’ nutritional status, weight trends, intake, and overall wellness.
• Maintain accurate nutrition documentation within the electronic medical record system.
• Participate in interdisciplinary care plan meetings.
• Partner with Dining Services to support appropriate meal planning and service.
• Serve as a nutrition resource for facility leadership and clinical teams; mentor Diet Technicians and interns as applicable.
• Conduct nutrition-related audits and performance improvement initiatives.
• Maintain CMS/DOH compliance and support staff education and survey readiness.
